Lichtenstein versus Onstep for inguinal hernia repair: protocol for a double-blinded randomised trial.

چکیده
INTRODUCTION Inguinal hernia is a common condition that affects millions of people world-wide every year. In Denmark (population of 5.5 million), more than 10,000 repairs of inguinal hernias are performed annually. The optimal surgical procedure for mesh placement and fixation is still being debated because of long-term complications such as persisting pain and impairment of sexual function. The Onstep approach is a newer type of groin hernia repair with promising preliminary results in terms of very few cases of chronic pain and recurrences. This protocol describes a randomised clinical trial the objective of which is to evaluate chronic pain and sexual dysfunction after inguinal hernia repair using the Lichtenstein repair compared with the Onstep approach. MATERIAL AND METHODS The study is designed as a two-arm blinded multicentre, randomised clinical trial, currently involving five centres in Denmark and with ongoing recruitment. The plan is to recruit a total of 282 patients (Lichtenstein, n = 141 and Onstep, n = 141) and to perform one-year follow-ups. Follow-up will be done by clinical examination, phone interviews and questionnaires. CONCLUSION This study will be the first randomised clinical trial to compare the Lichtenstein repair with the Onstep technique. The results are important in order to guide further research and clinical guidelines for inguinal hernia repair. FUNDING The study was funded in part by Bard Europe. TRIAL REGISTRATION NCT01753219 (clinicaltrials.gov).

ONSTEP versus laparoscopy for inguinal hernia repair: protocol for a randomised clinical trial.
INTRODUCTION The optimal repair of inguinal hernias remains controversial. It is recommended that an inguinal hernia be repaired using a mesh, either with a laparoscopic or an open approach. In Denmark, the laparoscopic approach is used in an increasing number of cases. The laparoscopic repair has a learning curve of about 50-100 cases and decreases chronic pain, but slightly increases the risk...
متن کاملLong-Term Follow-Up of a Large Series of Patients Following ONSTEP Inguinal Hernia Repair
Purpose: Long-term (3-5 years) results following inguinal hernia repair in a large series of patients using a novel technique-the ONSTEP approach-are presented. In particular, the recurrence rate, long-term complications and patient satisfaction with the procedure are discussed. Methods: Adult patients underwent ONSTEP inguinal hernia repair using a PolySoftTM hernia patch. All procedures were ...

Background: The inguinal hernia is a common disorder in general surgery. Different methods have been described for repair of these hernias. In modern methods, synthetic mesh is used to cover the wall defect and the most known method is Lichtenstein surgical repair. The laparoscopic totally extra peritoneal procedure (TEP) is a newer technique of repairing hernia. The aim of this study is to co...
متن کاملThe Tilburg double blind randomised controlled trial comparing inguinal hernia repair according to Lichtenstein and the transinguinal preperitoneal technique
BACKGROUND Anterior open treatment of the inguinal hernia with a tension free mesh has reduced the incidence of recurrence and direct postoperative pain. The Lichtenstein procedure rules nowadays as reference technique for hernia treatment. Not recurrences but chronic pain is the main postoperative complication in inguinal hernia repair after Lichtenstein's technique. Preliminary experiences wi...
متن کاملComparing non-mesh and sutured inguinal hernia repairs in groin surgery: A randomized clinical trial
Purpose: Mesh technique is the standard for inguinal hernia repair because of less recurrence, but it is inferior or equal to sutured technique in case of other post-operative complications such as chronic pain. This clinical trial set out to compare these two techniques. Materials and Methods: A total number of 322 cases of unilateral inguinal hernia in participants older than 18 yea...
